The Building Healthy Communities (BHC) Program
Foundations For Health believes health care services should be accessible for all Australians, regardless of your financial situation. As such, Foundations For Health is committed to providing a set number of subsidised telehealth appointments per month under the BHC program for people whose financial situation would prevent them from accessing the service at the standard rates^.
All consultations are charged at $35 each under the program, regardless of duration.
BHC Program Fortnightly Appointment Allocation
Foundations For Health will provide the following telehealth appointments* under the BHC program:
1 x Initial Consultation (75 mins) per fortnight
2 x Follow Up Consultations (length determined on a case by case basis) per fortnight
These appointments may only be booked in advance until the end of the next calendar month. Once all allocated appointments are booked for this time period, anyone wishing to access an appointment under the program will be placed on a waiting list.
At the start of each calendar month, any clients on the waiting list will be offered new available appointments according to the date they were added to the wait list. If there are no clients on the wait list, the appointments will remain free until a booking request is made.
Any unused appointments will not be rolled over to the next fortnight/month.

* Appointments will be reduced during school holiday times to reflect the reduced operating hours of the clinic. No initial consultations will be offered under the BHC program during school holidays.

Terms and Conditions
Due to the limited availability of appointments under the BHC program, clients must adhere to the following T’s & C’s in order to access the program:
At this stage, the BHC program is open to Australian residents (currently living in Australia) only.
The BHC program is available for low income health care card holders experiencing financial hardship.
Online bookings cannot be made under the scheme, all bookings must be made directly with the practitioner.
Each client is eligible for up to 3 appointments under the program per calendar year. All 3 appointments must be used within 6 months of the initial appointment.
All consultations must be paid in full at the time of booking to secure the appointment under the BHC program.
All consultations booked under the BHC program must adhere to Foundations For Health’s standard Cancellation Policy.
If a client misses an appointment, or cancels/reschedules an appointment within 48 hours of the arranged time twice within a 6 month period, they will no longer be eligible for the program at any time in the future.
Multiple clients (e.g. siblings, family members etc) cannot share an appointment, all consultations within the program are for individual consultations only.
Clients must demonstrate a commitment to implementing the agreed treatment plan to maintain eligibility for the program.
If at any time Foundations For Health deems the therapeutic relationship between the client and the practitioner to be untenable, we maintain the right to decline to continue the therapeutic relationship.
Clients who have access to an alternative fee subsidy program (e.g. NDIS) are ineligible for the BHC program, ensuring the availability of appointments for those who do not have access to an alternative support network.
