Privacy Policy

The purpose of the privacy policy is to provide information on how we use and protect personal information about individuals, and to describe the ways Foundations For Health (hereby abbreviated to FFHealth for the remainder of this document) adheres to the Privacy Act 1988, in conjunction with the Australian Privacy Principles.

1.    Collection of Information Personal information such as your name, date of birth, contact details, payment details and sensitive health information will only be collected for purposes necessary to the functions and activities of FF Health, including the provision of health services.

 Personal information will only be collected by means that are permissible by law.

 2.    Use of Information Your personal information will be collected, held, used and/or disclosed for the following purposes:

  • Provision of services you have engaged us to provide, including but not limited to, personal health consultations services, online workshops & education services and group coaching/mentoring services.

  • To maintain our business records.

  • To keep in touch with you.

  • For any purpose required or permitted by law.

  • For any purpose you would otherwise reasonably expect.

  • For any purpose disclosed to you and which you have provided consent for.

  • To otherwise run our business.

 3.    Disclosure of Information FFHealth will not use the information for purposes other than that for which it was collected unless the client has provided their consent; the use of the information for alternative purposes is necessary to prevent or lessen a serious and imminent threat to the life or health of the client to whom the information relates, or of another person; or, if so directed by a court of law.

 If FFHealth uses or discloses personal information (authorised above), a written note identifying the purpose and reason for its disclosure will be recorded.

 4.    Access to Information FFHealth will provide the client access to their personal health record upon written request, except in the case where access to the information could pose a serious and imminent threat to the life or health of any client; providing access would be likely to prejudice an investigation of possible unlawful activity; denying access is required or authorised by or under law; and, providing access would be likely to prejudice the outcome of an internal investigation. FFHealth reserves the right to recoup any costs associated with the provision of a client’s personal health record.

 FFHealth will not disclose personal information to anyone of any 3rd party organisation, unless the disclosure is necessary to provide a service the client has requested; prevent or lessen a serious and imminent threat to life or health of any person; in relation to an investigation or law enforcement process; and where legislation recognises lawful access by some government agencies.

 5.    Security of Information FFHealth will take reasonable steps to protect the personal information it holds from misuse and loss, and from unauthorised access, modification or disclosure.

 6.    Questions & Grievances Please contact FFHealth immediately using the email address in point 8. of the Procedure section below, should you have any questions relating to this privacy policy, if you believe we have not complied with our legal obligations, or if you believe the security of the personal information we hold has been compromised in any way. We will respond to your question or concern as soon as possible, however if your you are dissatisfied with our response you may direct a complaint to the Office of the Australian Information Commission by email at enquiries@oaic.gov.au or by phoning 1300 363 992.

7.    Changes to this Privacy Policy At any time, FFHealth may change or update this Privacy Policy without prior notice to you. The current and most up-to-date Privacy Policy will be available online, however if you would like to request a copy to be sent to you please contact use at any time.

 Procedure

1.      Any requests for client health information must be made in writing, with identification provided, and directed to the Business Owner for actioning.

2.      Client case information will not be visible, either on paper or digital device, to another client or external party.

3.      Client case information will not be discussed in front of another client or external party.

4.      Anonymous client case information may be discussed with other industry professionals only for the benefit of the client care with the information shared limited to that necessary for effective case management.

5.      All sensitive health information will be kept filed in a secure filing cabinet or uploaded to the client’s digital health file held in our clinic software program, with the paper copy subsequently destroyed in accordance with legislative requirements.

6.      Personal information collected for the provision of other services such as workshops & education services, group coaching/mentoring services and provision of email communications relating to FFHealth’s services will be either be uploaded and stored digitally, or locked in a secure filing cabinet.

7.      Access to all digital health records will be restricted using password protected health practice software. Devices displaying client case information will not be left unattended for any reason. In the event that the service provider is required to leave the device unattended, they will logout of the health practice software.

8.      FFHealth may use your personal information to provide information which may interest you, including but not limited to, third party research, or other products and services. You may unsubscribe from marketing communications at any time by directly emailing us at KellyRobinson.FFHealth@gmail.com, or by other suitable methods such as clicking the ‘unsubscribe’ button where applicable.

9.      In the event a client’s personal information has been, or reasonably suspected to have been, subjected to a breach of privacy, the Business Owner will notify the client as soon as possible after the breach has been detected.

 Website Use

1.    Non-personal Information In addition to any personal information mentioned above, when you use this website we may collect other, non-personal identification information from you, such as your browser name, type of device you are using, the operating system of your device and other similar information.

2.    Web Browser Cookies During your use of this site we may use “cookies” to gain information from you relating to your use of this site. This information may include, but is not limited to, functional cookies, performance cookies and tracking cookies. We recommend all users to review the cookie settings on their browser and customise them in line with their own personal preferences. Please note, that some functionality of this website may be reduced based on your cookie preference settings.

3.    Third Party Programs, Plug-ins and Software Both FFHealth clinical & online services, along with this website, uses a variety of plug-ins and third party software to provide enhanced services and functionality, including but not limited to, Cliniko, Squarespace, MailChimp, Zoom, Gmail, Vital.ly and Oborne Health Supplies. FFHealth has no control over the individual privacy policies of these third party companies; by using these third party plug-ins and software you signify that you are in agreeance with the conditions set by these providers. FFHealth recommends all users to contact these third party service providers directly to determine their privacy policy and Ts&Cs before making use of any of their associated products/services.
